The objective of this study was to evaluate the effects of varying the anchorage modalities during maxillary molar distalization using Carrière Distalizer appliance. Materials and Methods: Twenty eight patients indicated for bilateral maxillary first molar distalization were selected from the out-patient clinic of Orthodontic Department, Faculty of Dentistry, Minia University. Patients were randomly divided into two equal groups .Distalization was performed with the Carriere distalizer appliance in both groups. In the first group, miniscrews between mandibular second premolar and mandibular first molar was utilized for anchorage. In the second group, full arch lower fixed appliance with lingual holding arch was used as anchorage source. One cone beam computerized tomographic scan was taken for all patients before distalization and an additional one after a bilateral Class I molar relationship was attained in the same standardized technique. Measurements were made and statistically analyzed to compare the amounts of molar distalization, anchorage loss and skeletal changes between both groups. Results: There was statistically significant difference in the amount of maxillary first molar distalization between the miniscrews group (MS) and full fixed appliance (FFA) group (3.06±1.22 mm, 2.01±1.09 mm, subsequently). There was statistically significant difference in antero-posterior position of mandibular first molar between MS group and FFA group (0.08±0.02 mm, 0.7±0.46 mm, respectively).Also, there was a statistically significant difference in bucco-lingual inclination of lower incisor between both groups (1.02±0.55° in MS group and 2.3±1.17° in FFA group with p value of 0.045).
